1. What the game is about

Animal Hospital is an anomaly-detection and survival game set inside a hospital. Your job is to check in patients, identify whether they are normal or anomalous, treat normal patients, and turn away suspicious patients before they become a threat.

Core loop: receive a patient → inspect appearance → use photos or cameras if needed → decide whether they are safe → treat normal patients and reject anomalies.

2. First things to do

  • Learn the front desk. You spawn in the check-in area, which contains the camera monitor, phones, coffee machine, trash bin, and some optional items.
  • Watch your Sanity. Sanity works like health. If it reaches 0%, you die. The default starting Sanity is about 61%.
  • Use coffee to recover. Coffee at the front desk restores Sanity and has a 180-second cooldown after you take one.
  • Do not rush check-ins. Most early mistakes happen when a player lets an anomaly into the hospital too quickly.

Medical Wing, Rooms 1-5

Standard diagnosis rooms for DNA extraction, analysis, diagnosis, and treatment.

Emergency Wing, Rooms 6-8

Special rooms with extra minigames.

  • Room 6, X-Ray Room: copy the flashing color sequence to finish the scan.
  • Room 7, Heart Monitor Room: click the correct buttons until the monitor reaches 100%. The red skull button lowers progress.
  • Room 8, Surgery Room: finish a timed 45-second surgery by using the requested tools. Do not shoot or stun the Surgery Monster, because that can kill the patient.

4. Choosing a class

Classes are small modifiers that can be bought with Animal Coins or Robux. Your equipped class appears above your avatar in the lobby and next to your Sanity meter during the round.

5. How to detect anomalies

Use this order: visual check → photo check → camera check. The earlier you catch an anomaly, the safer the run becomes.

Visual anomalies

  • Oversized eyes or an unusually wide smile
  • Three glowing red eyes and a distorted voice
  • No eyes and an unnatural frown
  • Mismatched eyes with a creepy smile
  • Twitching hands or head
  • Realistic teeth with large eyes

Photo anomalies

  • Bloodshot eyes and a creepy grin in the photo
  • Different ears or eyes in the photo
  • Distorted or noisy photo
  • Realistic teeth and large eyes visible in the photo

Warning: some photo anomalies cost 10 Sanity when you pick up and inspect the photo. Avoid repeated checks when your Sanity is low.

Camera anomalies

  • Distorted body on camera
  • Black rectangle with eyes in front of the patient’s head
  • Patient appears completely black on camera
  • Patient appears as a Skinwalker on camera
  • Patient looks directly at the camera
  • Mismatched ears, twitching hands, or twitching head only visible through the camera

If the camera view starts zooming in on an anomaly, leave the camera quickly or you may lose Sanity.

Bed Monster

Appears under a bed and creates a red danger zone. Bring maple syrup before entering the red zone.

Head Banger

Give it coffee to make it leave. Sending it away without coffee costs 20 Sanity.

Skinwalker

Often comes from letting an anomalous patient into the hospital. Reject suspicious patients early.

Hiders

If they chase you, run in the opposite direction until they disappear. They can steal items and lower Sanity.

Stalker

A tall black rabbit-like figure that appears around corners. Looking directly at it costs 10 Sanity.

Surgery Monster

Complete the surgery before the timer ends. Do not use the gun or taser on it.

Camera Figure

Appears when viewing a broken camera. Exit the camera quickly, or the scare can cost 20 Sanity.

7. Beginner route

Inspect every patient in the lobby before accepting them.

If the patient looks normal, use photos or cameras when you are unsure.

If any check shows an anomaly, reject the patient.

4.

If the patient is safe, send them to the correct room and complete the diagnosis or treatment task.

5.

Return to the front desk for coffee when Sanity gets low.

6.

In solo play, carry only high-value items and avoid filling your inventory with low-priority supplies.

8. Quick rule to remember

Look first, photograph second, check cameras third. If something feels wrong, reject the patient. If Sanity is low, drink coffee. In emergency rooms, focus on finishing the minigame instead of fighting.
